What this means for you: When you open an account, we will ask for your name, address, date of birth and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.

Commodity Trading Advisor ("CTA")
If the account will be traded by someone other than the account owner, and the trader is registered as a Commodity Trading Advisor ("CTA").
Discretionary Account
If the account will be traded by someone other than the account owner, and the trader is NOT registered as a Commodity Trading Advisor ("CTA").
Non-US Customer
If the account is owned by a non - US customer.
Transfer
If the account is currently with another brokerage company and the owner wishes to transfer the account to FCStone, LLC.
Authorization of Delivery of Statements by Electronic Media
Email and fax consent form in PDF format.
Futures Direct Account
If the account is being opened as a Futures Direct Account.
Specialized Brokerage Account
If the account is being opened as a FCStone's Specialized Brokerage Account.
